Home Care Membership with Hope Bridge Home Health is designed to establish care prior to any emergent need. Through the Membership program, clients can complete all necessary paperwork and receive a thorough assessment with our Clinical Care team. The assessment will allow our Clinical Care team to establish a plan of care that will be valid for a period of six (6) months from the date of the assessment. HopeBridge will provide services, per the plan of care, at the current fixed and hourly rates. Rates are subject to change; clients will be notified 30 days in advance of any rate increases.

Membership fees are non-refundable and non-transferrable. The membership fee is $200 per client or $260 per couple living under the same roof, and can be renewed every 6 months.

The client will be contacted prior to the 6-month membership renewal to schedule a re-assessment visit. The client has the option of renewing the membership at that time by scheduling the re-assessment and paying the membership fee that will be valid for an additional six (6) months. The membership fee may be waived if a minimum of 40 hours of fulfilled and paid shifts were completed in the previous six months and no major changes in healthcare status have occurred. The client also has the option of discontinuing services and not renewing the membership at no penalty.

It is the responsibility of the client to report any hospitalizations or healthcare changes to HopeBridge. Updated documentation is necessary to provide proper care. Changes in healthcare status, as with a new diagnosis or injury, may result is a new assessment with the Clinical Care team prior to the 6 months re-assessment/renewal date. If a new assessment is needed, client will be charged the current hourly rate for Care Management.

The client (or POA) will be responsible for completing all necessary paperwork, paying the Membership fee, and understanding the terms of the membership and service agreement prior to any scheduled shifts.

Membership includes the following benefits:
Preferred Scheduling:
Members can call our scheduling line to set up a visit with one of our trained caregivers. Scheduled visits are a minimum of three hours in length and subject to our scheduled block times. Visits must be scheduled 8 days in advance to avoid over-time charges. Preferred scheduling allows visits to be arranged regularly or on an as-needed basis.
On-Call Urgent Care:
Members needing more emergent care (visit needed less than 8 days in advance) have access to on-call staff subject to caregiver availability. Overtime charges may occur for these visits.
Post-Surgical Support:
Members have access to care staff to help after discharge from the hospital or rehab facility. Support includes help understanding discharge instructions, transportation, meal prep and assistance with activities of daily living. Scheduled visits should be made 8 days in advance to avoid over-time charges. 24-hour Nursing Support:
Access to our Clinical Care team is available 24 hours a day to answer questions. This is NOT a substitute for emergency medical services; the client should still call 911 for emergencies.
Respite Care:
In-home respite care is available to members with a minimum of 15 days’ notice. Respite care is ideal for families leaving town for vacations, etc. and unable to care for or check in on loved ones.
Errands and Appointments:
Care staff may be available to run errands for or with clients, or attend and/or transport for appointments. Shifts may be less than the 3-hour minimum and are subject to staff availability. Attending appointments with members can be ideal to ensure that important information is not missed.
Event Companionship:
Members can utilize care staff for support and companionship for community or family events. This may include transportation and stability assistance during an event. Companionship visits should be scheduled a minimum of 8 days in advance to avoid overtime charges.
